Urgent appointments
To ask for an urgent same day appointment (Monday to Friday),
SystmConnect is available for urgent requests from 8am to 3pm when we are open
- phone us on 02392 736006, Monday to Friday, when we are open
- visit the Medical Centre and speak with a receptionist, Monday to Friday, when we are open
For all new conditions please fill in an online form through ‘SystmConnect’
To make sure that you receive an urgent Face to Face appointment on the same day, please contact us before 12 noon.
You will need to give our Care Navigators with as much information as possible.
We will use the information to select the most suitable clinician or service to help you.
We call this Care Navigation.

Routine appointments
To ask for a routine appointment for all ongoing conditions,
- phone us on 02392 736006, Monday to Friday, when we are open
- visit the Medical Centre and speak with one of the Receptionists, Monday to Friday, when we are open
- To ask for Fit notes, Doctors letters and more medication please fill in an online form:
If you are unable to submit your request online, then please call us on 02392 736006 and speak to a Care Navigator.
When you get in touch, we will ask what you need help with.
We will use the information you give us to choose the most suitable clinician to help you.

Your appointment
We may offer you a consultation:
- by phone
- face to face at the surgery.
Appointments by phone, or video call can be more flexible, this may mean the clinician may be able to see you sooner.
The clinician may also be able to assess your condition without speaking to you. In these circumstances they will text or email you.
To cancel your appointment:
- Use your NHS account (on the NHS website or in the NHS app).
- visit the Medical Centre and speak with a receptionist, Monday to Friday, when we are open
- reply CANCEL to your appointment reminder text message
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
Please tell us:
- If there’s a specific clinician you would prefer to respond.
- If you would prefer to consult with the clinician by phone, face-to-face, by video call or by text or email.
- If you need an interpreter.
- If you have any other access or communication needs.
If you are housebound and need an appointment, we will do a home visit. We will phone you first to understand what you need.
To request a home visit, it’s helpful if you phone the practice as soon after 8am as possible on 02392 736006.
