Urgent appointments
To request an urgent appointment for today (Monday to Friday)
- complete an online form through SystmConnect
- phone us on 02392 736006, Monday to Friday, when we are open
- visit the medical centre and speak with one of the receptionists, Monday to Friday, when we are open
For all new conditions please complete an online form through ‘SystmConnect’
To ensure that you receive an urgent Face to Face appointment on the same day, we kindly request that you contact us before 12 noon.
You will need to provide our Care Navigators with as much relevant information as possible.
We will use the information that you give us to choose the most suitable clinician or service to help you. This is called Care Navigation.

Routine appointments
To request a routine appointment for all ongoing conditions
- please complete an online form through eConsult. This service is available Monday to Friday from Monday to Friday from 6.30am to 8.30am. We will respond during opening times
If you are unable to submit your request online then please call us on 02392 736006 and speak to a Care Navigator
When you get in touch, we’ll ask what you need help with.
For Fit notes, Doctors letters and Medication requests please complete an online form through ‘SystmConnect’
We will use the information you give us to choose the most suitable clinician to help you.

Your appointment
However you choose to contact us we may offer you a consultation:
- by phone
- face to face at the surgery
- on a video call
Appointments by phone, video call or by text or email can be more flexible and often means the clinician can help you sooner.
To cancel your appointment:
- Use your NHS account (on the NHS website or in the NHS app).
- visit the Medical Centre and speak with a receptionist, Monday to Friday, when we are open
- reply CANCEL to your appointment reminder text message
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
Please tell us:
- If there’s a specific clinician you would prefer to respond.
- If you would prefer to consult with the clinician by phone, face-to-face, by video call or by text or email.
- If you need an interpreter.
- If you have any other access or communication needs.
If you are housebound and need an appointment, we will do a home visit. We will phone you first to understand what you need.
To request a home visit, it’s helpful if you phone the practice as soon after 8am as possible on 02392 736006.
